What is an All-on-4 Treatment?

The All-On-4 concept is an reconstructive dental technique that provides a permanent solution to create function and aesthetics in cases of multiple missing teeth or badly broken down teeth. This treatment can replace an entire jaw of missing teeth, which can be 8-10 teeth, with four dental implants instead of having one implant for each missing tooth!

With All-on-4 treatment, ALL teeth are placed on 4 dental implants. In some cases where more stability is desired, 6 dental implants may be useful (And dental treatments will be called All-on-6).

The procedure involves strategically placing four dental implants in the jaw bone. A custom-made dental prostheses containing a full set of teeth for the jaw is then placed on top of the implants, providing you with a set of beautifully aligned and functional teeth.

1. Dental Implant Consultation

2. Smile Design

Once you have decided, it is time to start planning! We start by taking a virtual scan of your teeth. This would create an accurate 3D model of your teeth.

With the help of your 3D x-ray scans, our dentist will assess your bone structure and identity the anatomical structures present. That way, we are able to be accurate with our procedure while minimising risks.

Additionally, you have the opportunity to select the colour and shape of how your new teeth will be, or you may trust our experts to choose a natural appearance for you. This preparation allows us to digitally design your smile before surgery, tailoring it to your preferences.

3. All-on-4 Surgery and Temporary Artificial Teeth

On the day of the surgery, local anaesthetic will be given to make the process comfortable and any remaining hopeless teeth, if present, will be removed. The dental implants will be placed accurately in the pre-planned positions using our implant surgical guide. The surgical site will then be covered and the gum tissue held together by sutures.

On top of the implants, the dentist will secure the temporary teeth, allowing you to leave our clinic with a functional set of teeth on the same day while the implants integrate with the jawbone during the healing phase.

4. Review Check

Our dentist will see you in two weeks to check on the progress of your implants, possibly remove any stitches and make any necessary adjustments to your artificial teeth.

Complete healing may take anywhere from 3-6 months after the surgery and it is common to experience some discomfort during this healing phase.

To promote good healing, it is recommended to adhere to the instructions given to you and inform your dentist of any sores, ulcers, looseness or any unexpected uncovering of the dental implants. This healing phase is important, as the implants needs time to fuse to your jawbone and stabilise.

5. Permanent Artificial Teeth and Final Adjustments

After the healing process, a permanent bridge is prepared to replace the temporary bridge. Using CAD/CAM software, the shape of the new bridge is designed.

You have the option between two materials for your implant-supported permanent bridge: Acrylic or Ceramic. Each material has its own characteristics and benefits (read more about this below). Your dentist will discuss these options with you in detail to help you choose a suitable material for your needs.

After the permanent bridge is fabricated, your dentist will check the colour and fit of the bridge. With your approval, the bridge is attached in place, completing the final process.

Alternatives to All-on-4 Treatment

  1. Conventional Complete Removable Dentures: The conventional complete dentures rest completely on your gums. They are less stable and less comfortable than implant-supported dentures and All-On-4, which can affect the way you talk and eat. Additionally, while they are affordable, they do not look as natural or function as well. They also need to be removed daily for cleaning and maintenance.

  2. Traditional Implant-Supported Dentures/Overdentures: This approach is similar to the All-on-4 concept, but it involves placing two to eight vertically positioned implants in each jaw instead of four. Adding more implants can extend the treatment duration which contributes to higher overall costs and more visits to the dental clinic for the patient. Additionally, the artificial teeth can be either fixed permanently inside the mouth or designed to be removed by the patient for cleaning and maintenance.
